The Chanda Center for Health (CCFH/the Center) is a non-profit organization in Lakewood, Colorado, with a mission to improve the health and wellness of people living with long-term disabilities and reduce their associated healthcare costs. We believe that individuals with long-term physical disabilities have the right to create their own healthcare path, live independently, and be active members of the community.

Position Summary

The Center is hiring a part-time (24 hours/week) Care Coordinator to help our participants navigate the complexities of accessing treatments available at the Center, as well as connecting them with resources in the community. This participant-facing role helps manage new participant intake, eligibility, care planning, and documentation.

Success in this position requires strong organizational skills, clear and compassionate communication, and a proactive approach to follow-up and problem-solving. The Care Coordinator plays a vital role in supporting our mission to improve access to integrative health services for people with long-term physical disabilities.

Essential Duties and Responsibilities

Intake & Enrollment

  • Respond to service inquiries and conduct eligibility screenings across all referral types
  • Enter new participant information and manage intake documentation using the electronic health record (EHR) system
  • Partner with external agencies (i.e. Case Management Agencies, Single Entry Point entities, County Human Services) to support participant enrollment and access to services, including eligibility verification, application coordination, and documentation support across various referral types and funding sources
  • Develop and submit initial care plans to appropriate agencies for approval
  • Conduct facility tours, complete care coordination assessments, and support participants in setting personalized goals

Resource Referral & Support

  • Serve as the main point of contact for participant inquiries, offering guidance and support in accessing services and achieving personal goals
  • Collaborate with internal and external partners to plan, schedule, and promote resource-related events and educational sessions
  • Address service-related issues such as denials, appeals, and reinstatements, and escalate concerns to external agencies when needed

Outreach & Participant Growth

  • Conduct outreach and community engagement efforts to grow and maintain the participant base
  • Represent the Center at relevant events and meetings
  • Reconnect with eligible inactive participants to encourage re-engagement
  • Prepare and distribute regular participant communications

Program Tracking & Reporting

  • Monitor and maintain accurate participant and program data using the Center’s EHR and reporting tools
  • Track key performance indicators (KPIs) and participate in team reviews to identify trends and improve processes
  • Collaborate with leadership to align data collection with reporting needs and strategic goals

Qualifications

Required Qualifications

  • Minimum of 2 years’ experience in care coordination, case management, or a related human services field
  • Experience supporting individuals in navigating healthcare, human services, or community-based resources
  • Proficiency in using Electronic Health Record (EHR) or Client Relationship Management (CRM) systems
  • Proficiency with Microsoft Office Suite and cloud-based systems
  • Excellent communication and interpersonal skills, with a commitment to delivering high-quality customer service to participants, staff, providers, and external partners
  • Deep alignment with the mission of the Chanda Center for Health and sensitivity to the needs of individuals living with long-term physical disabilities

Preferred Qualifications

  • Bachelor’s degree in human services, social work, public health, or a related field
  • Familiarity with Medicaid, Medicaid Waiver programs, and/or services for individuals with disabilities
  • Prior experience with community outreach, health navigation, or advocacy within the disability community
  • Knowledge of local and state-level systems that support individuals with long-term physical disabilities

Working Environment/Physical Activities:

  • Extended periods of sitting, often near a computer.
  • Fast-paced and active work environment.
  • Must be able to lift 50 pounds independently or with assistance.

Work Hours: Your normal work schedule will be 24 hours per week, Monday through Friday. Monday is a required workday, and two additional days will be assigned. Each workday includes an unpaid meal break, in accordance with Colorado labor regulations.  Because this position is non-exempt, you would be eligible for overtime.

Workplace: Onsite at the Chanda Center for Health (1630 Carr Street, Lakewood, CO 80214) with some ability to work from home. Travel off-site to meetings and events will be necessary.
